By electronically typing my name below, I give permission for my child to participate in activities with the 381 Youth Collective. I understand that reasonable precautions will be taken for safety, but in the event of an accident or injury, I will not hold 381 Youth Collective, participating churches, or leaders liable. I give permission for the staff or volunteers to seek emergency medical care for my child if necessary.


I give permission for my child to attend off-site activities organized by the 381 Youth Collective, including transportation provided by church vans, rented buses, or adult volunteers approved by the Collective or Partner Churches. I understand that reasonable safety measures will be taken and that all drivers will be adults with valid licenses and insurance. I release the 381 Youth Collective, participating churches, and leaders from liability in the case of injury or incident related to transportation or off-site events. I understand I will be notified in advance of such events.

We have reviewed and agree to the behavioral expectations outlined by the 381 Youth Collective. Students are expected to:

  • Refrain from profanity or obscene language
  • Not possess or use weapons, tobacco, vapes, alcohol, or drugs
  • Avoid bullying, harassment, or intimidation (physical, verbal, emotional, or online)
  • Avoid fighting or physical altercations
  • Avoid public displays of affection or any sexual behavior
  • Show respect to adult leaders and peers
  • Participate respectfully in group activities
  • Not vandalize, steal, or damage property
